Fluticasone propionate includes a class of drugs known as corticosteroids, especially glucocorticoids, which are hormones that predominantly affect carbohydrate metabolism and, to a lesser extent, fat and protein. It is used to treat asthma, allergic rhinitis, nasal polyps, various skin disorders and Crohn's disease and ulcerative colitis. It is also used to treat eosinophilic esophagitis.

Medical use

Fluticasone is used by inhalation powder or aerosol for asthma prophylaxis, nasal spray is used for prophylaxis and treatment of allergic rhinitis, nasal drops are used in the treatment of nasal polyps, and creams and ointments are applied topically in the treatment of various skin disorders. Can be administered orally in the treatment of Crohn's disease and ulcerative colitis. Several benefits are also reported in celiac disease.

Adverse effects

If taken correctly, nasal sprays and oral inhaler formulations have fewer corticosteroid side effects than tablet formulations because they limit the systemic (blood) absorption. However, systemic absorption can not be ignored even with the correct delivery. Using sprays or inhalers at doses higher than recommended or with other corticosteroids may increase the risk of systemic corticosteroid-induced side effects. These side effects include weakened immune system, increased risk of systemic infection, osteoporosis, and high pressure in the eyes.

Common side effects may include nasal irritation (burning, stinging, bleeding), headache, abdominal pain (nausea, vomiting), and diarrhea. Rare side effects include infection (proven by, for example, fever, sore throat, and cough), vision problems, severe swelling, hoarseness, and difficulty in breathing or swallowing.

Oral inhalation

Common side effects may include upper respiratory tract infection, throat irritation, canker sores, cough, and headaches. Rare side effects include bruising, facial/neck swelling, depression, fatigue, and shortness of breath.

Pharmacology

Fluticasone propionate is a highly selective agonist of glucocorticoid receptors with negligible activity in androgen receptors, estrogen, or mineralocorticoids, resulting in anti-inflammatory and vasoconstriction effects. It has been shown to have various inhibitory effects on different types of cells (eg mast cells, eosinophils, neutrophils, macrophages, and lymphocytes) and mediators (eg histamine, eikosanoid, leukotrienes and cytokines) involved in inflammation. Fluticasone propionate is said to provide a topical effect on the lungs with no significant systemic effect on the usual doses, due to low systemic bioavailability.

Interactions

Fluticasone propionate is broken down by CYP3A4 (Cytochrome P450 3A4), and has been shown to interact with strong CYP3A4 inhibitors such as ritonavir and ketoconazole.

Ritonavir is a common drug used in the treatment of HIV. Co-administration of ritonavir and fluticasone can cause increased levels of fluticasone in the body, which can lead to Cushing's Syndrome and adrenal insufficiency.

Ketoconazole, an antifungal drug, has also been shown to increase the concentration of fluticasone that leads to systemic corticosteroid side effects.
